IFCCS Coordinator Bilingual

About the Role:The IFCCS Coordinator Bilingual will play a critical role in coordinating services for clients referred to our IFCCS program. This includes maintaining contacts with DCFS and DMH, assisting with linkages to community resources, and coordinating team meetings with clients. The successful candidate will work collaboratively with therapists, staff, family members, public and private agencies, and community support systems to ensure comprehensive care for our clients.
Key Responsibilities:- Schedule and facilitate CFT meetings on a monthly basis.
- Develop and coordinate care plans with the IFCCS team.
- Maintain weekly contact with clients and their families.
- Consult with representatives from DCFS, DMH, and the child's attorney as necessary to coordinate care.
- Ensure that plans from system partners are integrated into the Matrix to address the goals and objectives of all parties involved in the care of the youth.
- Provide ICC and IHBS services as required or when clients are in crisis.
- Ensure outreach to new IFCCS referrals within 24 hours of receiving the referral and follow up with the Department of Mental Health Countywide Administration.
- Complete all Mental Health Progress notes within 24 hours of services being provided.
- Complete all other required IFCCS or Mental Health documentation within required time frames.
- Maintain 24-hour on-call availability, unless otherwise designated.
