Role Responsibilities:

The Case Manager for Transitional Age Youth will assist young individuals in acquiring and sustaining vital life skills as they navigate the transition from the children's care system to the adult care system. This role encompasses direct service crisis intervention, conducting intakes and assessments, providing client support, facilitating transportation, and offering educational resources to both individuals and families. Additionally, the Case Manager will be tasked with making referrals to partner organizations and community services based on the specific needs of clients. It is essential to ensure the delivery of high-quality services, including clinical care and documentation, while adhering to agency policies and maintaining all client records.

Qualifications:

  • A Bachelor's Degree in a Behavioral Health-related field is preferred or must meet Behavioral Health Technician requirements.
  • Experience working with children in a community behavioral health environment is advantageous.
  • Familiarity with case management processes and conducting intakes is preferred.
  • Possession of a Fingerprint clearance card or eligibility to obtain one is required.
  • A valid driver's license with no significant infractions in the past three years is necessary.
  • Regular access to reliable transportation is essential.
  • Bi-lingual proficiency in Spanish is preferred.
  • Applicants must be 21 years of age or older at the time of employment.

  • Ability to maintain composure in crisis situations, work independently and collaboratively, and demonstrate empathy towards individuals of diverse backgrounds.
  • Uphold professional conduct in alignment with ethical practices and agency policies.
  • Foster positive working relationships with internal and external team members, stakeholders, and community partners.
  • For roles involving driving, a valid driver's license and appropriate liability insurance are required.
