Our organization has been a trusted resource since 1987, dedicated to fostering positive outcomes for individuals and families.
Role Responsibilities:
The Case Manager for Transitional Age Youth will play a vital role in assisting young individuals as they navigate the transition from the children's care system to adult services. This position involves providing direct crisis intervention, conducting intakes and assessments, offering client support, facilitating transportation, and delivering educational resources to both individuals and families. Additionally, the Case Manager will be responsible for making referrals to partner organizations and community resources based on client needs. It is essential to ensure the delivery of high-quality services, including clinical care and thorough documentation, while adhering to agency policies and procedures.
Qualifications:
- A Bachelor's Degree in a Behavioral Health-related field is preferred or must meet Behavioral Health Technician requirements.
- Experience working with children in a community behavioral health environment is advantageous.
- Familiarity with case management processes and intake procedures is preferred.
- Possession of a Fingerprint clearance card or eligibility to obtain one is required.
- A valid driver's license with no significant infractions in the past three years is necessary.
- Access to reliable transportation is essential.
- Bi-lingual proficiency in Spanish is preferred.
- Candidates must be at least 21 years of age at the time of employment.
Additional Skills and Responsibilities:
- Ability to maintain composure in crisis situations, work independently and collaboratively, and demonstrate empathy towards individuals from diverse backgrounds.
- Uphold professional conduct in line with ethical standards and agency policies.
- Foster positive working relationships with internal teams, stakeholders, and community partners.
- If driving for agency purposes, a valid Arizona driver's license and appropriate liability insurance are required.
