About Us:

Role Responsibilities:

The Case Manager for Transitional Age Youth will play a pivotal role in assisting young individuals as they navigate the critical transition from child-focused care to adult services. This position encompasses a range of direct service activities, including crisis intervention, intake assessments, client support, transportation, and educational outreach to both individuals and families. The Case Manager will also facilitate referrals to partner organizations and community resources based on the unique needs of clients. It is essential to ensure the provision of high-quality services, including clinical care and accurate documentation, while adhering to all agency policies and procedures.

Qualifications:

  • A Bachelor's Degree in a Behavioral Health-related field is preferred, or candidates must meet Behavioral Health Technician requirements.
  • Experience working with children in a community behavioral health environment is highly desirable.
  • Prior experience in case management and conducting intakes is advantageous.
  • Must possess a Fingerprint clearance card or be eligible to obtain one.
  • A valid driver's license in Arizona with no significant infractions in the last three years is required.
  • Access to reliable transportation is necessary.
  • Bi-lingual candidates (Spanish) are encouraged to apply.
  • Applicants must be at least 21 years of age at the time of employment.
