Role Responsibilities:

The Case Manager for Transitional Age Youth will play a pivotal role in assisting young individuals as they navigate the critical transition from child-focused care to adult-oriented services. This position encompasses a range of direct service activities, including crisis intervention, intake processes, comprehensive assessments, client support, transportation, and educational outreach for both individuals and families. Collaboration with other in-home services and community partners will be essential to meet client needs effectively. The Case Manager will ensure the provision of high-quality services, including clinical care and thorough documentation, while adhering to all agency policies and maintaining accurate client records.

Qualifications:

  • Bachelor's Degree in a Behavioral Health-related field is preferred, or candidates must meet Behavioral Health Technician (BHT) requirements.
  • Experience in a community behavioral health setting, particularly with children, is highly desirable.
  • Familiarity with case management processes and intake procedures is advantageous.
  • Possession of a Fingerprint clearance card or eligibility to obtain one is required.
  • A valid driver's license in Arizona with a clean driving record over the past three years is necessary.
  • Access to reliable transportation is essential.
  • Bi-lingual proficiency in Spanish is preferred.
  • Candidates must be at least 21 years of age at the time of employment.

  • When required to drive for agency-related activities, a valid Arizona driver's license and appropriate insurance coverage are mandatory.
