The Family Service Association is seeking a compassionate Case Manager to join our Guardianship program. This full-time position entails a 35-hour work week, Monday through Friday, with opportunities for a hybrid remote schedule.

Location: The role primarily serves clients in the North Shore area of Massachusetts, covering Essex and Middlesex Counties.

Key Responsibilities
  • Conduct regular in-person visits with assigned adult clients (18 years and older) at their residences.
  • Document monthly progress notes and maintain records of collateral contacts.
  • Utilize personal reliable transportation for client visits, with occasional travel for supervision or team meetings.
  • Uphold the confidentiality of all program services and communications.
  • Ensure all necessary consents are obtained prior to any medical or psychiatric procedures for clients.
  • Participate in an on-call rotation to address emergency situations during evenings and weekends, as needed.
  • Collaborate with the fiscal department to ensure clients meet the income guidelines established by state regulations.
  • Report any financial discrepancies to the appropriate supervisory personnel and take necessary corrective actions.
  • Assist in managing clients' financial resources to ensure their needs are adequately met.
Qualifications
  • A Bachelor’s degree in Psychology, Social Work, Sociology, or a related field from an accredited institution, or equivalent experience.
  • Prior experience working with disabled or elderly adults in community or long-term care settings.
  • Familiarity with fiscal management or accounting related to client funds.
  • Demonstrated experience in case management, ensuring continuity of care for clients.
