Job Title: Clinical - Long Term Care Nurse Specialist (NY Only)

We are seeking a skilled Clinical - Long Term Care Nurse Specialist to join our team in Brooklyn, NY. This is a 03+ months contract opportunity with long-term potential.

Job Summary:

The successful candidate will be responsible for conducting visits to members' homes, reviewing EMR and previous assessments, and accurately documenting according to the provided documentation template. They will also utilize Lenavi to identify inconsistencies and complete and load supplemental tools and documents.

Key Responsibilities:
  • Prepare for visits by reviewing EMR and previous assessments.
  • Use critical thinking and problem-solving skills to address complex situations.
  • Travel to members' homes to conduct visits.
  • Accurately document according to the provided documentation template.
  • Review documentation and utilize Lenavi to identify inconsistencies.
  • Complete and load supplemental tools and documents.
  • Be flexible with schedule and process changes.
Requirements and Technology Experience:
  • Key Skills: MLTC, MAP, UAS
  • Complete 2-5 visits depending on the type of visit assigned.
  • Visits have different time allotments assigned based on what is required during the visit.
  • Individuals with MLTC, MAP or Maximus experience.
  • UAS RN.
  • Flexibility.
  • Excellent written and verbal communication.
  • Proficiency in Word, Outlook, Excel, Microsoft, Adobe, and EMS.
