Job Title: Clinical Nurse Specialist - Long Term Care

We are seeking a skilled Clinical Nurse Specialist to join our team in Brooklyn, NY. This is a contract opportunity with long-term potential.

Job Summary:

The Clinical Nurse Specialist will provide high-quality care to patients in their homes, utilizing critical thinking and problem-solving skills. The ideal candidate will have experience with MLTC, MAP, and UAS, and be proficient in Word, Outlook, Excel, Microsoft, and Adobe.

Key Responsibilities:
  • Prep for visits by reviewing EMR and previous assessments
  • Travel to patients' homes to conduct visits
  • Accurately document according to provided templates
  • Review documentation and identify inconsistencies
  • Complete and load supplemental tools and documents
Requirements and Technology Experience:
  • MLTC, MAP, or Maximus experience
  • UAS RN
  • Flexibility
  • Excellent written and verbal communication
  • Proficiency in Word, Outlook, Excel, Microsoft, and Adobe
