Hospice Liaison at Prestigious Hospital

We are seeking a skilled Hospice Liaison to join our team at one of NYC's most prestigious hospitals. As a Hospice Liaison, you will play a critical role in collaborating with physicians, medical staff, and clients to evaluate eligibility for hospice services.
Key Responsibilities- Collaborate with physicians, medical staff, and clients to evaluate eligibility for hospice services
- Communicate with discharge personnel to ensure a smooth and satisfactory transfer of patients to home or skilled facility settings
- Educate physicians, discharge planners, and social workers about agency services and the benefits of hospice care
- Prospect for and identify new physicians and discharge planners who have potential to refer patients to agency for hospice needs
- Conduct initial and ongoing needs assessments of patients in hospital and respond accordingly
- Attend and assist in enhancing effectiveness of liaison meetings and serve on agency committees
- Assist in creating printed educational materials and brochures regarding services provided and present material to patients
- Participate with other Health Care Liaison team members in co-marketing and coordination of hospice services
- Creates and maintains records and files of all referral contacts
- Meets and/or exceeds referral and admission quota expectations for assigned location onsite at Partner Hospital
- RN with active NY State Nursing license in good standing
- Minimum of two (2) years healthcare, home health, or hospice liaison experience preferred
- Knowledge of standards of practice for all services provided
