Job Title: RN - Hospital Liaison

United Hospice, Inc. is seeking a skilled Registered Nurse to join our team as a Hospital Liaison. This is an extraordinary opportunity to provide clinical and administrative oversight at our assigned hospital location.

Key Responsibilities:
  • Develop and maintain positive relationships with referral sources and promote exemplary customer service to physicians, facility staff, patients, and families.
  • Identify hospitalized nursing home residents who meet eligibility criteria to arrange for or conduct admissions to UH before discharge.
  • Collaborate with hospital case managers to review the hospital daily census and identify appropriate patients for UH admissions.
  • Admit patients to hospice as needed and serve as covering visit nurse for hospitalized UH patients in absence of regular staff.
  • Participate in on-call rotation to handle new referrals when office is closed.
Requirements:
  • Valid and current NYS Registered Nurse license; BSN preferred.
  • Two years recent clinical nursing experience with strong clinical skills including one year recent Med-Surg. experience required.
  • Ability to assess the needs of terminally ill patients and their families and to plan care to meet these needs.
  • Commitment to the Hospice Philosophy of Care.
  • Ability to relate to a diverse population and workforce in a personable and effective manner.
  • Excellent written and verbal communication skills.
  • Proficient in MS Office Suite, EMRs, and ability to learn new software systems.
