Hospice Supportive Care Nurse

At Hospice Of Hope, Inc, we are seeking a compassionate and skilled Hospice Supportive Care Nurse to join our team. As a key member of our care team, you will provide one-to-one care to terminally ill patients in their homes, nursing homes, and assisted living facilities in Kentucky and Ohio.
Responsibilities:- Provide end-of-life care to patients and their families, assessing and responding to their needs in a timely manner
- Develop and implement individualized care plans, collaborating with healthcare providers and other team members as needed
- Document patient information, using clinical tools and data to inform care decisions
- Communicate effectively with patients, families, and healthcare providers, utilizing strong written and verbal skills
- Travel to patient homes, nursing homes, and other facilities, using reliable transportation and adhering to universal precautions
- Graduate of an accredited School of Nursing, with current licensure in Kentucky and Ohio
- At least one year of experience in patient care, preferably in a hospital, nursing home, or related healthcare facility
- Personal qualities of empathy, responsibility, and dependability, with effective communication skills and ability to work independently
- Valid driver's license, current auto insurance, and reliable transportation
