Hospice and Palliative Physician Job Description

United Hospice is seeking a skilled Hospice and Palliative Physician to join our team. As a key member of our interdisciplinary team, you will provide medical care and leadership to our patients and their families.

Responsibilities:
  • Participate in the evaluation and recertification of patients' terminal prognosis at regular intervals.
  • Collaborate with our interdisciplinary team to develop, implement, and revise the physician plan of care (PPOC).
  • Complete documentation of all patient care services provided.
  • Intervene when necessary and review revocations with the IDT.
  • Provide medical expertise to admission and patient care staff on issues and challenges involving terminal prognosis and pain and symptom management.
  • Review patients' medication regimens for adverse reactions, inappropriate usage, and duration.
  • Collaborate with attending, referring, and/or inpatient physicians on issues and challenges involving terminal prognosis and pain and symptom management.
  • Provide home visits based on the agreed-upon plan of care and as directed by the Clinical Director.
  • Provide general medical needs of patients to the extent that these needs are not met by the attending physician.
  • Provide family members with information about the medical management and prognosis of the patient within HIPAA guidelines.
  • Provide medical care within Hospice regulations and UH guidelines, policies, and procedures.
  • Review and determine hospice eligibility for patients referred to Hospice with an oncological primary terminal diagnosis.
  • Facilitate up-front communication with referral sources to ensure seamless hand-off and recommend consultation with UH Homebased Palliative Care team when appropriate.
  • Provide medical expertise on issues and challenges involving evaluation of terminal prognosis and pre-admission symptom management needs for hospice patients.
  • Work closely with internal staff and external referral sources to identify areas of opportunity to increase hospice utilization for patients with an oncological primary terminal diagnosis through education and innovation.
  • Provide physician leadership to UH partners to facilitate a seamless transition to hospice for patients who are under the care of community hospitals and meet hospice eligibility criteria.
Qualifications:
  • Licensed and currently registered to practice medicine or osteopathy in New York State required.
  • DEA license required.
  • Board-eligible/certified in Internal Medicine, Family Practice, or other related specialty with a working knowledge of the principles of palliative medicine and symptom management required.
  • A valid driver's license may be required, as determined by operational/regional needs.
  • CPR (Cardiopulmonary Resuscitation) Certification is preferred.
  • PECOS enrollment is required.
Education:
  • Medical Degree required, MD.
