Job Description

The LPN Lead is responsible for assisting the physician and staff with the daily coordination of patient care and physician access.

The LPN Lead functions as the liaison between patients, staff, and physicians.

Essential Job Functions:
  1. Responsible for providing professional nursing care to office patients in noncomplex situations under the supervision of Physician and Clinical Practice Manager or Clinical Coordinator.
  2. Delivers direct patient care within the scope of licensure and in accordance with SMG policy.
  3. Assumes accountability for the quality of care, which he/she has delivered, including maintaining standard precautions.
  4. Collects lab specimens according to departmental guidelines. Accurately labels specimens.
  5. Prioritizes patient care by implementing physician orders and nursing procedures.
  6. Documents appropriately in the Electronic Health Record (EHR).
  7. Assures all patients are correctly identified prior to providing and/or documenting care.
  8. Communicates & provides care consistent with age, cultural, spiritual, and developmental needs of the patient.
  9. Maintains efficient patient flow.
  10. Employs appropriate and timely use of Tasking in EHR.
  11. Schedules appointments appropriately and accurately according to department guidelines.
  12. Administers injections according to appropriate protocol. Documents injections accurately in EHR.
  13. Appropriately demonstrates telephone triage protocols and document in EHR.
  14. Effectively communicates problems, concerns, or issues to the Office Supervisor/Manager appropriately and promptly.
  15. Ensures proper preparation for patient visit. Prepares pertinent information needed for patient visit.
Obtains Vital Signs:
  1. BP, temp, pulse, height & weight accurately and completely as required. Documents appropriately in EHR.
  2. Reviews and updates medication list in EHR. Demonstrates understanding of prescription control and prescription refill procedures. Accurately and completely documents known allergies in EHR.
  3. Appropriately enters orders and schedules appointments in the Practice Management System (PMS) and EHR.
  4. Provides referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
  5. Appropriately adheres to universal safety precautions when administering medications and disposal of medical waste.
  6. Maintains proficiency with department-specific equipment, troubleshoots, and reports malfunctions promptly.
  7. Facilitates transition of care to UCC/Hospital.
  8. Checks and maintains logs for emergency equipment according to policy.
  9. Assists in ordering and maintaining of all supplies as assigned.
  10. Demonstrates flexibility with various work schedules.
  11. Recognizes and performs duties which need to be performed although not directly as.
General Job Functions:
  1. Responsible for maintenance of staff and provider schedules in Shift Hound. Work with Office Supervisor/Clinical Coordinator to provide adequate staff coverage.
  2. Responsible for oversight of medical and office supplies and ordering of supplies.
  3. Monitor employee performance of staff assigned to office. Discuss concerns and issues with Office Supervisor/Clinical Coordinator/Clinical Practice Manager.
  4. Responsible for addressing and resolving patient complaints/issues. Communicate issues and resolution to Providers, Office Supervisor/Clinical Coordinator/Clinical Practice Manager. Bring concerns expressed by providers to Office Supervisor/Clinical Practice Manager.
  5. Notify Office Supervisor/Clinical Coordinator/Clinical Practice Manager of any potential health and safety hazards in the office. Responsible to place and follow through on Facilities/IT work orders related to the office setting.
Education, Certification, Computer, and Training Requirements:
  1. Vocational / Technical School / Diploma Program required. Associates degree preferred.
  2. New Jersey State Nursing License and BLS certification required.
  3. 0 - 1 years experience. 2-4 years experience preferred.
  4. Ability to communicate in English, both orally and in writing.
  5. Strong interpersonal and organizational skills.
  6. Ability to prioritize and organize multiple tasks in a timely manner.
  7. Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  8. Experience with Standard Office Technology in a Window-based environment preferred.
  9. Experience with patient care equipment preferred.
  10. Experience with EHR, Practice Management System, and Departmental/Ancillary Systems preferred.
