Program Overview:

The Healthcare-Community Partnerships (HCP) Capacity Building team leverages best practices from PHS' extensive experience in healthcare collaborations to implement scalable projects aimed at improving health access and equity. This team is instrumental in integrating health-related social needs (HRSN) screening and referrals into diverse healthcare settings, including hospitals and primary care facilities.

Role Summary:

We are seeking a knowledgeable Practice Improvement Specialist to spearhead quality enhancement initiatives within our Quality Improvement Learning Collaboratives (QILCs). This role will focus on assisting healthcare partners in implementing HRSN screening processes and ensuring effective navigation of resources to meet identified needs. Key responsibilities include:

  • Delivering tailored technical assistance to healthcare partners, including coaching and resource development.
  • Creating quality improvement materials and tools to bolster healthcare partner capabilities.
  • Monitoring and evaluating the use of QI resources by healthcare partners.
  • Analyzing quality measures data to guide improvement strategies.
  • Facilitating learning sessions and collaborating with team members to develop training programs.
  • Fostering strong relationships with healthcare partner teams to support continuous improvement.

Qualifications:

  • Bachelor's degree in Public Health or a related field; a Master's degree is preferred.
  • 2-3 years of experience in quality improvement, capacity building, or a related area.
  • Experience in supporting quality improvement processes within healthcare settings is highly desirable.
  • Familiarity with public health systems and healthcare provider networks is advantageous.
