Field Health Advocate Specialist

- To foster collaborative relationships with healthcare teams, community partners, and staff members to achieve comprehensive care planning and positive health outcomes.
Responsibilities:
- Conduct thorough assessments of members' health status, identifying care needs and developing individualized care plans.
- Collaborate with interdisciplinary teams to coordinate access to internal and community-based resources, ensuring seamless care transitions and continuity.
- Document all case activity accurately and efficiently, utilizing CHPW's care management system to track progress and evaluate outcomes.
Requirements:
- Bachelor's degree in nursing or master's degree in social work and/or related behavioral health field (preferred).
- Current, unrestricted license in Washington State as registered nurse (RN), Social Worker (LSWAA, LSWAIC), Mental Health Counselor (LMHC), Mental Health Professional (LMHP), or Marriage and Family Therapist (LMFT) required.
- Minimum one year experience in case management, home health, or discharge planning; or equivalent combination of education and experience.
- Three years of clinical experience in acute care and/or outpatient setting required.
- Proficiency and experience with Microsoft Office products.
Compensation and Benefits:
This exempt position offers a competitive salary range of $63,000 - $83,000 per annum, plus benefits, including medical, dental, vision, 401(k) retirement plan, and generous employer match. The base pay offered will take into account internal equity, job-related knowledge, skills, and experience among other factors.
