About the Role

This Social Services Specialist I position plays a critical role in delivering person-centered care to our patients, working closely with our multidisciplinary team to address complex social determinants of health.

Key Responsibilities
  1. Conduct comprehensive psychosocial assessments to identify client strengths and needs.
  2. Develop tailored intervention plans that address identified gaps in care.
  3. Coordinate referrals to community resources and support services as needed.
What We're Looking For
  • Bachelor's degree in a relevant field and current certification as a counselor or chemical dependency counselor.
  • Proven experience working with vulnerable populations, including those with substance use disorders and mental health challenges.
  • Strong knowledge of community resources and networks to facilitate whole-person care.
