Key Responsibilities

The Primary Care Provider will deliver quality healthcare through direct patient interactions in the clinic and via mediated communication channels, such as telephonic consultations, video calls, or online messaging. They will collaborate closely with Regional Directors, Clinic Directors, Supervising Physicians, and all clinic staff, including telehealth team members, to ensure excellent patient care. Additionally, the Primary Care Provider will assist in coordinating care referrals to support the patient's overall health.

  • Conduct comprehensive physical examinations and assessments for patients of all ages, including children aged 3 years and older.
  • Manage chronic care for patients with conditions such as hypertension, diabetes, and asthma.
  • Perform annual wellness visits for both adults and children, including vaccinations and preventive screenings.
  • Order, interpret, and evaluate diagnostic tests to assess patients' clinical issues and healthcare needs.
  • Collaborate with physicians and other healthcare professionals to create comprehensive patient care plans.
  • Prescribe medications, recommend treatments, and provide detailed discharge instructions.
  • Refer patients to specialists or additional healthcare resources as necessary.
  • Educate patients and their families on health maintenance and preventive care strategies.
  • Encourage patient engagement, health literacy, and shared decision-making.
  • Participate in quality improvement initiatives and value-based care programs.
  • Focus on patient-centered care, continuity of care, and building strong patient-provider relationships.
Experience and Qualifications
  • Nurse Practitioner (NP) or Physician Assistant (PA) with a minimum of 2 years of dedicated experience in a primary care setting.
  • Proficient in delivering comprehensive primary care, including chronic care management and preventive health measures.
  • Strong understanding of Patient-Centered Medical Home (PCMH) principles and value-based care.
  • Willingness to conduct pediatric examinations and care for children aged 3 years and older.
Educational Requirements

Master's Degree in a relevant field.
