We are seeking a motivated and compassionate Patient Health Navigator to join our team and contribute to our mission of improving the health and well-being of our patients. As a Patient Health Navigator, you will play a critical role in helping patients navigate the healthcare system and access the care they need.

Some key responsibilities of this role include:

  • Scheduling appointments and navigating patients through health system barriers
  • Initiating communication with patients and assisting with support in making referrals to address their full spectrum of needs
  • Participating in mandatory trainings, service projects, and team-building activities
  • Creating informational materials and available resources for Sea Mar sites
  • Collaborating with Sea Mar medical clinic staff, quality improvement staff, and other Sea Mar departments as needed

Your salary for this position is approximately $24,000 per year, and you will receive a stipend/living allowance of $24,000 prorated over the course of your service. You will also be eligible for a maximum Education Award of $7,395 after successful completion of your service time.

To be considered for this position, you must have excellent verbal and written communication skills, including the ability to compile, interpret, and utilize information, and to communicate effectively with people of all backgrounds and educational levels. You must also be able to work with minimal supervision, use good judgment, meet deadlines, and prioritize workloads. Reliable transportation is required.
