About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Community Health Specialist. In this role, you will work closely with individuals with Autism Spectrum Disorder and Serious Mental Illness, providing clinical services and coaching supports to help them maintain stability and functionality in their homes and communities.

Key Responsibilities
  • Conduct psychiatric assessments and physical health needs assessments
  • Make appropriate referrals to community physicians and provide management and administration of medication
  • Collaborate with the team, individuals, and their collaterals to develop and revise comprehensive assessments and individualized treatment plans
  • Manage pharmaceuticals and medical supplies
  • Provide clinical and non-clinical support, including psychoeducation, educational and vocational supportive counseling, and symptom management
Qualifications
  • Registered Nurse licensed in Massachusetts
  • Minimum of 3 years experience working with individuals with psychiatric issues, ASD, and other developmental challenges
  • Valid Drivers License and reliable transportation
