Job Title: Community Health Worker

We are seeking a dedicated and passionate Community Health Worker to join our team at Worcester RISE for Health. This role will focus on supporting newly arriving refugee and migrant families, with a particular emphasis on pregnant individuals and infants under 5.

Key Responsibilities:
  • Conduct outreach and engagement activities to identify and support pregnant individuals and families in need of health navigation services.
  • Build trusting relationships with community members to facilitate open communication and provide education on various health topics.
  • Assist individuals in navigating the healthcare system, including addressing health insurance challenges and connecting to primary care practices.
  • Connect clients to social services and resources to address social determinants of health.
  • Organize and participate in health promotion activities and events within the community.
Requirements:
  • Associated or Bachelor's degree in a related field.
  • Previous experience in community health, social work, or related fields is preferred.
  • Passion for maternal child health.
  • Strong communication and interpersonal skills.
  • Cultural sensitivity and the ability to work with diverse populations.
  • Knowledge of local healthcare resources and services.
  • Ability to work independently and as part of a team.
  • Proficiency in basic computer skills – specifically Word, Excel, and Email.
  • Proficiency in Haitian-Creole.
