About the Role

Careforth is seeking a highly experienced Senior Manager to lead our Care Coordination team. As a key member of our leadership team, you will be responsible for driving the delivery of high-quality care coordination services to our clients.

Key Responsibilities
  • Oversee the implementation and evaluation of clinical programs and care coordination models to ensure compliance with contractual, regulatory, and NCQA accreditation requirements.
  • Manage departmental budgets and support expected fiscal performance through care management processes and practices.
  • Lead, coach, and mentor staff to promote a culture of continuous quality improvement, cost reduction, and improved outcomes.
  • Develop and implement quality and performance improvement initiatives to drive clinical, quality, and fiscal outcomes.
  • Collaborate with the Director, Care Management, to drive the delivery of high-quality care coordination services.
Requirements
  • Bachelor's degree or equivalent in a healthcare or social services discipline.
  • Minimum 7+ years of progressive experience in care/disease management, working with complex populations, specifically older adults and/or individuals with disabilities.
  • 5+ years of management experience.
  • Comprehensive knowledge/experience in managing Medicaid services, particularly LTSS services.
  • An active RN license is strongly preferred.
  • Ability to travel within assigned geographic area.
