Job Summary

We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at Yale New Haven Health. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions of care for our patients.

Responsibilities
  • Coordinate placement of hospitalized patients with extended care facilities, home care, or other community-based agencies.
  • Facilitate delivery of equipment and supplies as directed by Care Coordinator or Social Work partner.
  • Collaborate with Care Coordinators to monitor and prioritize discharge plans for each patient.
  • Ensure all necessary paperwork and electronic documentation is completed and available for patient discharge.
  • Identify and report any issues causing delays or lack of timely discharge.
Requirements
  • Associate Degree and/or Bachelor of Science Degree in Business Administration, Human Services, Health Administration, or other healthcare-related field.
  • Two to three years of experience in a healthcare environment.
  • Excellent communication and organizational skills.
