Job Title: Community Care Coordinator

Join Sound Community Services, Inc. as a Community Care Coordinator and play a vital role in empowering individuals with persistent mental illness and substance use disorders to achieve recovery and wellness.

Job Summary:

We are seeking a highly motivated and compassionate individual to provide a complete network of aftercare services for individuals transitioning to independent living in the community. As a Community Care Coordinator, you will work closely with clients, families, and community resources to develop and implement personalized recovery plans, ensuring a smooth transition to community-based care.

Key Responsibilities:
  • Provide service delivery, including contact hours and location of services, as dictated by the DMHAS Fidelity Scale.
  • Document functional assessments, recovery plans, and encounter notes thoroughly, in a timely manner, and in line with the DMHAS Fidelity Scale.
  • Demonstrate an understanding and appropriate use of Stages of Change, Motivational Interviewing, and Skills-Based Interventions.
  • Participate in team meetings, attend meetings prepared to discuss relevant client issues and treatment plan goals, and work collaboratively with the Client Accounts Administrator to support Persons Served.
  • Identify the need for reduced level of care, ensuring a timely transition to Recovery Pathways.
  • Facilitate yearly reassessments as necessary.
  • Attend mandatory PSRB Acquittee Fundamental Training Program and refresher courses in accordance with DMHAS requirements.
  • Identify the need to discharge from services in a timely manner, ensuring documentation is completed, and maintain records of client contact and updates in the electronic client record.
  • Develop and review recovery plans with Persons Served and assigned team members.
  • Provide advocacy and crisis intervention services as indicated by the treatment plan.
  • Develop linkages and follow-up programs for other agency-based support services and/or other community resources.
  • Support families as needed in understanding the impact of mental illness.
  • Assure appropriate entitlements are in place and assist PS in completing necessary paperwork to apply or reapply for benefits.
Requirements:
  • Associate degree in Human Services, Social Work, Education, Psychology, or a related field.
  • Minimum of two (2) years providing care management services to individuals with behavioral health needs.
  • Current, valid Motor Vehicle Driver's license and automobile available for use during the workday required.
  • Competent use of technology, including email, agency intranet, and the electronic client record.
  • Demonstrated ability to work with clients in the field, teaching skills necessary for community living and supporting recovery.
  • Excellent written and oral communication skills.
  • CPR/First Aid certified or eligible.
