Case Manager and Coach for Homeless Recovery and Mental Health

Mental Health America of Los Angeles (MHALA) is a pioneer in mental health service, advocacy, innovation, and training. We are seeking a skilled Case Manager and Coach to join our team and support individuals in their recovery journey.
The successful candidate will work closely with our team to engage and enroll new members, develop personalized recovery plans, and provide coaching and advocacy to help individuals achieve their goals. This is a rewarding opportunity to make a meaningful difference in the lives of others.
Key Responsibilities- Engage and enroll new members, developing personalized recovery plans that identify their needs and goals
- Advocate on behalf of members to help them access housing, healthcare benefits, and other essential services
- Visit members in their homes, hospitals, or jails as needed to support their recovery and prevent homelessness
- Transport members to appointments and errands when necessary
- Provide coaching, counseling, and mentoring to help members develop independent living skills, such as money management, grooming, and hygiene
- Develop strategies to reach out to members who tend to isolate themselves
- Provide timely information and feedback to the program manager regarding emergent issues
- Assist members and the team in achieving community integration, education, and employment goals
- High school diploma or GED required
- Minimum of one year of experience providing psychosocial rehab services to a disabled population required
- Bachelor's degree in a human-related field preferred
- Valid driver's license, vehicle, and insurance required
- Ability to work in a team-oriented approach to member services
- Knowledge of harm reduction philosophy and motivational interviewing preferred
- Valid CPR certificate preferred
- Knowledge of computer applications such as Microsoft Word, Excel, and Outlook
- Flexible work schedule to include some evenings or weekends, and on-call cell phone services when necessary
