Roots Through Recovery is seeking a skilled Case Manager to join our team. As a Case Manager, you will work with clients and families to manage and overcome substance abuse and co-occurring issues. You will utilize assessments and self-assessments to map out client wellness and recovery plans, serving as an accountability partner and coach.
Key Responsibilities- Direct work with clients and families to manage and overcome substance abuse and co-occurring issues
- Utilize assessments and self-assessments to map out client wellness and recovery plans
- Hold clear boundaries and communicate directly and effectively with clients and their families
- Develop strong, healthy relationships with clients to support and challenge them to change and thrive
- Teach and model hard and soft life skills
- Maintain fluency in supportive community resources and develop strong working relationships with community treatment and clinical service providers
- Exhibit high levels of self-awareness and continually assess performance, identify challenges and needs, and develop areas of opportunities
- Maintain complete and accurate records
- Model a growth mentality and engage in personal growth work in parallel with clients and families
- A minimum of a High School Diploma or Equivalent is required
- Minimum 2 years Case Management experience working in the drug and alcohol treatment and/or mental health treatment industry
- Experience working with private insurance and Drug Medi-Cal is a plus
- Knowledge of recovery tools, recovery resources, motivation, and behavior, as well as coaching as a change agent
- Certified as a Drug and Alcohol Counselor or a degree in Social Work
- Bilingual candidates will be highly considered
- Expertise in addiction recovery, relapse prevention, community resources, legal issues, workforce development, and coaching
- Ability to navigate and roll with resistance and deploy techniques to enhance motivation to change
- Experience in compassionate crisis intervention and ability to de-escalate clients
- Ability to serve as a role model, positive mentor, and support person
