About the Role

Mental Health America of Los Angeles is seeking a dedicated Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals experiencing homelessness and mental health challenges. Your primary responsibility will be to engage and enroll new members, develop personalized recovery plans, and advocate on their behalf to secure housing, healthcare benefits, and other essential services.

Key Responsibilities:

  • Engage and enroll new members, developing a comprehensive recovery plan that addresses their unique needs and goals.
  • Mediate and advocate on behalf of members to secure housing, healthcare benefits, and other essential services.
  • Visit members in their place of residence, hospital, or jail as needed to support their successful transition and prevent homelessness.
  • Transport members to appointments and errands when necessary.
  • Monitor and oversee members' individual and group activities, providing coaching, counseling, and mentoring to promote independent living skills.
  • Develop and implement strategies to reach isolated members and provide timely information and feedback to the team leader.
  • Assist members and the team in achieving community integration, education, and employment goals.
  • Participate in staff and community meetings, contributing to the day-to-day operations of the program.

Requirements:

  • High school diploma or GED required.
  • Minimum one year of experience providing psychosocial rehabilitation services to a disabled population.
  • Bachelor's degree in a human-related field preferred.
  • Valid driver's license, vehicle, and proof of insurance are conditions of employment.
  • Ability to demonstrate tolerance, patience, and understanding for individuals with multiple needs and issues.
  • Knowledge of harm reduction philosophy and motivational interviewing.
  • Flexible work schedule, including evenings and weekends, and on-call cell phone services when necessary.
  • TB clearance and re-testing every year are conditions of continued employment.
  • COVID-19 vaccination is required for this position.
