Case Manager for Homeless Housing Recovery and Mental Health Services

Mental Health America of Los Angeles is seeking a dedicated Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals experiencing homelessness and mental health challenges. Your primary responsibility will be to provide intensive case management services, helping clients achieve stability and independence.
Key Responsibilities- Engage and enroll new members, developing personalized recovery plans to address their unique needs and goals.
- Advocate on behalf of clients to secure housing, healthcare benefits, and other essential services.
- Visit clients in their homes, hospitals, or other settings to provide support and prevent homelessness.
- Transport clients to appointments and errands as needed.
- Monitor clients' progress, providing coaching, counseling, and mentoring to promote independent living skills.
- Develop strategies to reach isolated clients and provide timely information to team leaders regarding emerging issues.
- Collaborate with clients and the team to achieve community integration, education, and employment goals.
- Participate in staff and community meetings, contributing to the day-to-day operations of the program.
- High school diploma or equivalent required; bachelor's degree in a human services field preferred.
- Minimum one year of experience providing psychosocial rehabilitation services to a disabled population.
- Valid driver's license, reliable vehicle, and proof of insurance required.
- Ability to work in a fast-paced, unstructured environment with a high level of tolerance, patience, and understanding.
- Knowledge of harm reduction philosophy and motivational interviewing techniques.
- Team-oriented approach to member services and ability to develop good community relations.
- Valid CPR certification and knowledge of computer applications (Microsoft Word, Excel, Outlook) preferred.
- Flexible work schedule, including evenings and weekends, and on-call cell phone services as needed.
