Social Services Coordinator

We are seeking a dedicated and compassionate Social Services Assistant to join our team at Kei-Ai Los Angeles Healthcare Center. As a key member of our Social Services Department, you will play a vital role in ensuring the emotional and social well-being of our residents.
Responsibilities- Assist the Director of Social Services in planning, organizing, and implementing social services programs.
- Coordinate transportation and appointments for residents, ensuring timely and efficient care.
- Provide residents and families with information on Medicare, Medicaid, and financial assistance programs.
- Maintain accurate social service progress notes and update the social services directory.
- Collaborate with community social health and welfare agencies to meet the needs of our residents.
- Protect resident confidentiality and maintain HIPAA standards.
- High school graduate or equivalent required.
- Previous experience in skilled nursing preferred.
- Strong communication and multitasking skills.
- CNA certification preferred.
- Valid California Driver's license in good standing.
