Social Services Coordinator

At National Health Foundation, we are seeking a highly skilled and compassionate Social Services Coordinator to join our team. As a key member of our post-hospital care services team, you will play a critical role in supporting individuals experiencing homelessness as they transition out of acute care hospitals.
Key Responsibilities:
- Conduct comprehensive needs assessments and develop personalized care plans to ensure seamless transitions for our guests.
- Facilitate discharge processes and coordinate housing placements to ensure our guests have a safe and stable place to call home.
- Collaborate with hospitals and healthcare providers to ensure timely and effective communication.
- Develop and maintain relationships with community resources and service providers to ensure our guests receive comprehensive support.
- Provide education and support to our guests on topics such as healthcare, housing, and employment.
- Conduct regular check-ins with guests to monitor progress and provide ongoing support.
- Collaborate with our nursing team to execute weekly case conferencing for guests.
- Develop and maintain accurate and up-to-date records and databases to track guest progress and program outcomes.
Requirements:
- Bachelor's degree in social work or a related field.
- Minimum of three years of experience in healthcare delivery, hospital education, or social services.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Strong organizational and time management skills.
- Proficiency in MS Office and ability to learn new software and systems quickly.
Physical Requirements:
The duties of this position require frequent standing and walking, as well as extended sitting. The employee will frequently bend/stoop, squat, reach above the shoulders, twist, and turn, kneel, and push/pull. The employee will frequently be required to lift/pull/push/move up to 50 pounds. Must utilize conversational speech for effective verbal communication. Noise level is high in some client care areas. Must be able to hear equipment alarms, overhead pages, and direct verbal communication.
