Job Description
The Service Coordination Navigator plays a critical role in supporting young adults as they navigate their career paths. As a Service Coordination Navigator, you will be responsible for connecting young adults with community resources, providing ongoing support, and assisting in creating transition plans. You will also administer life skills assessments and enter case notes into a database.

The ideal candidate will have excellent communication skills, be able to relate well to young adults, and have a passion for helping others. They will also be responsible for conducting outreach to justice and community organizations to receive client referrals.
