Job Overview

This is a part-time opportunity to work as a Care Manager supporting families and their loved ones through the healthcare system.

Primary Responsibilities:

  • Patient Assessment: Conduct comprehensive patient assessments and develop individualized care plans.
  • Service Coordination: Coordinate services such as transportation, meals, and household assistance.
  • Advocacy: Advocate on behalf of patients to ensure access to quality care.
  • Community Resources: Connect patients with community resources and support services.

Qualifications:

  • Education: Bachelor's degree in a healthcare-related field or equivalent experience.
  • Skills: Excellent communication, organizational, and problem-solving skills.
  • Licensure: Valid driver's license and preferred licensure in a related field (RN, SW, or LCSW).

Compensation: The estimated salary for this role is $55,000 - $65,000 per year.
