Care Coordinator

Salary and Benefits
The estimated salary for this position is $18.70 per hour, commensurate with experience. Our comprehensive benefits package includes health, dental, and vision insurance, as well as paid time off and opportunities for professional growth and development.
Responsibilities
As a Care Coordinator, you will be responsible for developing and implementing personalized care plans that meet the unique needs of each client. This will involve working closely with families, healthcare professionals, and other stakeholders to ensure seamless coordination of services.
Requirements
To be successful in this role, you will need to have at least 2 years of experience working with individuals with developmental disabilities, preferably in a community-based setting. A high school diploma or equivalent is required, as well as a valid driver's license. Additionally, you will need to possess excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
