Care Coordination and Advocacy Opportunity

MultiCare Health System is seeking a highly skilled Hospital Social Worker to join our team. As a Care Coordinator, you will play a vital role in ensuring seamless transitions of care for our patients.

About the Role:

  • Collaborate with healthcare teams to coordinate patient care across various settings.
  • Provide psychosocial assessments and interventions to support patients' mental and emotional well-being.
  • Develop and implement personalized care plans to address patients' unique needs.
  • Communicate effectively with patients, families, and healthcare professionals to ensure continuity of care.

Requirements:

  • Masters degree in Social Work from an accredited institution.
  • Licensed Independent Clinical Social Worker (LICSW) or Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
  • A minimum of three years of experience in hospital or equivalent settings.
  • Excellent communication and interpersonal skills.
