Patient Care Coordinator

MedStar Health seeks an experienced Registered Nurse to coordinate patient care and facilitate discharge planning. This role involves collaborating with healthcare teams, evaluating patient needs, and ensuring effective care delivery from pre-admission through post-discharge.
About the Role:
As a RN Case Manager-Discharge Planner, you will be responsible for coordinating patient care and facilitating discharge planning. This includes communicating with patients, families, and healthcare teams to address medical appropriateness to care coordination and discharge planning. You will also ensure the care is designed to promote optimal clinical and resource outcomes for acute and post-hospital phases of care.
Responsibilities:
- Communicates with patient, family and/or significant other, health care team, external case manager, community resources, and facility to address the ability to develop a plan of care that addresses needs across the short-term goals with specific time frames for resolution; services to be provided in the care plan; include the family/care-giver in the plan of care; and show life planning contingencies such as power of attorney
- Evaluates and documents the patient's response to the plan of care and care as indicated. Adheres to all policies and procedures regarding treatment, including psychosocial and financial concerns as well as medical.
Requirements:
- Associate's degree in Nursing (ADN) Required
- Bachelor's degree in Nursing (BSN) Preferred
- RN Case Manager, Discharge Planner, acute care, patient-centered care, case management, healthcare coordination, clinical expertise, care delivery, hospital discharge, multidisciplinary collaboration
Estimated Salary:$83,000 - $110,000 per year based on experience.
