Role Responsibilities:


The Hospice RN will operate within the guidelines of the Ohio Nurse Practice Act, delivering compassionate care tailored to the unique physical, emotional, social, and spiritual needs of each patient.

This role involves providing hospice care primarily in patients' residences.

The Hospice RN will act as a care navigator for patients receiving General Inpatient Care (GIP) in hospitals.

This position requires travel within the Central Ohio region.

Key Duties:

  • Nursing Process: Conduct thorough assessments and evaluations of patient care for a designated group, employing evidence-based practices. Collaborate with patients and families to create and implement comprehensive care plans that address physical, spiritual, and educational needs.
  • Communication: Ensure timely and accurate communication of essential information to relevant team members. Keep patients and families informed throughout the care process.
  • Documentation: Maintain precise and complete records of assessments, interventions, and patient outcomes in accordance with established standards and guidelines.
  • Delegation: Take responsibility for the care provided by both licensed and unlicensed personnel, ensuring accountability for all documented care.
  • Technology: Utilize technology effectively and engage in continuous learning to enhance clinical skills.
  • Assist in onboarding new staff and support the educational experiences of nursing students as appropriate.
  • Stay current with clinical knowledge, skills, and licensure, attending required training and meetings to promote professional development.
  • Participate in quality improvement initiatives that involve collaboration across departments.

Qualifications:

  • Experience: A minimum of two years of professional nursing experience is required.
  • Must possess a valid driver's license from the State of Ohio and have access to a reliable vehicle during work hours.
  • Current clinical experience in community home health or hospice settings is preferred.
  • Completion of a criminal background check and fingerprinting as mandated by Ohio law.
  • Education: Graduate from an accredited nursing program. A bachelor's degree in nursing is preferred.
  • Licensure: Must hold a current nursing license to practice in the state of Ohio.
