Hospice Registered Nurse - Care Coordinator
Mount Carmel Health System is actively looking for a Full-time Hospice Registered Nurse - Care Coordinator to become part of our dedicated team. As a member of Trinity Health, we are committed to delivering outstanding patient care with sensitivity and compassion. In this role, you will act as a navigator for hospice care, focusing on the physical, emotional, social, and spiritual needs of patients who are receiving General Inpatient Care.
Key Responsibilities:
- Conduct assessments and evaluations of patient care, formulating and executing a thorough care plan.
- Effectively communicate relevant information to team members and keep patients and families informed.
- Document interventions and outcomes in accordance with established standards.
- Delegate care tasks as appropriate to ensure comprehensive patient support.
- Stay updated on clinical practices and engage in performance improvement initiatives.
Required Qualifications:
- A minimum of 2 years of nursing experience is required.
- A valid driver's license and access to a personal vehicle with insurance are necessary.
- Current nursing license in Ohio is mandatory.
- Graduation from an accredited nursing program; a BSN is preferred.
- Experience in community home health or hospice environments is advantageous.
