Position Overview

Work Schedule: This role requires availability every other weekend along with two weekday shifts, including a holiday on-call rotation.

Key Responsibilities:

  • Conduct pre-admission assessments to determine service suitability.
  • Initiate the admission process by educating patients and families about available services.
  • Secure informed consent from patients and families.
  • Provide comprehensive nursing assessments, develop initial care plans, and implement interventions aimed at enhancing patient and family comfort and quality of life.

Qualifications:

  • Graduation from an accredited nursing program
  • Current registered nurse licensure in the applicable state
  • Valid CPR certification
  • Preferred: At least one year of experience in hospice care
  • Ability to make independent decisions as needed
  • Strong interpersonal skills for interacting with patients, families, and other stakeholders
  • Familiarity with quality assessment and assurance practices
