  • This is a Full-Time position, operating Monday through Friday from 8 AM to 4:30 PM, with responsibilities including holiday rotations.

Essential Responsibilities of the RN Care Coordinator:

The RN Care Coordinator is responsible for delivering high-quality care to patients and their families in various settings. This includes planning and executing care strategies based on comprehensive assessments, interventions, and evaluations. You will engage effectively with patients, families, and interdisciplinary team members while upholding the highest standards of professional nursing practice.

Qualifications:

  • Active Registered Nurse license in the State of Ohio without any disciplinary actions.
  • A minimum of two years of acute care nursing experience is preferred. Certification in Hospice & Palliative Medicine (CHPN) is preferred and must be obtained within two years of employment.
  • Current CPR certification.
  • Proficient computer skills for accurate documentation of services and care.
  • Ability to drive safely in various conditions.
  • Valid Driver's License with a clean driving record.
  • State Minimum Automobile Insurance Coverage.
