Role Summary
The on-site outpatient Clinical Nutrition Specialist will be tasked with evaluating the nutritional needs of patients and formulating evidence-based dietary care plans for individuals with diverse health conditions, including but not limited to Type 2 diabetes, hypertension, hyperlipidemia, metabolic syndrome, and weight management challenges. This role requires collaboration with the interdisciplinary team within a patient-centered medical home (PCMH) framework, ensuring comprehensive care delivery.
Key Responsibilities
- Conduct thorough nutrition assessments utilizing the nutrition care process to identify patient needs and recommend appropriate interventions.
- Provide nutrition education and health promotion initiatives to clients and support staff in various programs.
- Facilitate effective communication among the care team to ensure cohesive patient care plans.
- Evaluate the impact of nutrition-related medications and dietary supplements on patient health.
- Develop and review educational materials related to nutrition and preventive health strategies.
- Engage in quality assurance activities in compliance with federal guidelines.
- Collaborate with clinical teams for data collection and health outcomes reporting.
- Participate in professional development opportunities to enhance skills and knowledge.
Essential Qualifications
- Four-year degree in Dietetics or a related field, with preferred experience in community or clinical nutrition.
- Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentials.
- CPR and AED certification for healthcare professionals.
- Strong customer service skills and ability to work with diverse populations.
- Bilingual in English and Spanish is highly preferred.
