POSITION SUMMARY
This on-site outpatient Clinical Nutrition Specialist will be tasked with evaluating the dietary needs and formulating evidence-based nutrition care strategies for patients of all ages facing various health challenges, including but not limited to Type 2 diabetes, hyperlipidemia, hypertension, metabolic syndrome, and weight management.

COLLABORATIVE WORK ENVIRONMENT
This role is integral to the interdisciplinary team within a patient-centered medical home (PCMH) model, working alongside Health Educators, Nurses, and Medical Assistants to enhance quality improvement initiatives focused on body mass index (BMI).

QUALIFICATIONS
  • A four-year degree in Dietetics or a related field, with a preference for two years of experience in community or clinical nutrition education or completion of a dietetic internship with RD/RDN credentials.
  • Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentials from the Commission on Dietetic Registration.
  • Certification in CPR and AED for Healthcare Professionals through recognized courses.
  • Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety.
  • Proven excellence in customer service with the ability to engage and instruct individuals at various skill levels.
  • Demonstrated proficiency in nutrition education techniques and methodologies.
  • Familiarity with Microsoft Office applications and relevant program-related software.
  • Experience working with diverse populations and cultures.
  • Experience in motivational interviewing.
  • Valid Arizona driver's license and current auto insurance.
  • Ability to maintain confidentiality and demonstrate strong prioritization and multitasking skills.
  • Bilingual (English/Spanish) preferred.
  • Willingness to travel to multiple locations as needed.

POSITION RESPONSIBILITIES
  • Utilizes the nutrition care process to conduct comprehensive assessments of patients' dietary needs, recommending suitable nutrition therapy interventions and documenting care plans in electronic health records.
  • Provides nutrition education and health promotion to clients and collaborates with the Health Education team on preventative strategies.
  • Effectively communicates and coordinates relevant patient information among the care team to ensure continuity of care.
  • Evaluates and educates patients regarding nutrition-related medications, dietary supplements, and potential drug-nutrient interactions.
  • Develops, reviews, and updates nutrition-related educational materials and guidelines for health promotion services.
  • Educates staff on nutrition guidelines as necessary.
  • Refers patients to appropriate interprofessional team members and resources as needed.
  • Assists in completing required reports and conducts quality assurance in accordance with federal guidelines.
  • May establish relationships with external healthcare organizations to identify additional resources.
  • Collaborates with clinical teams in data collection, health outcomes reporting, and program evaluation.
  • May provide telehealth education using various communication platforms.
  • Participates in ongoing professional development through training and conferences.
  • Employs excellent customer service and communication skills in all interactions.
  • Performs other duties as assigned.
