Position Summary
This on-site outpatient Clinical Nutrition Specialist will play a vital role in evaluating the nutritional health of patients and creating evidence-based dietary care plans for individuals of all ages facing various health challenges, including but not limited to Type 2 diabetes, hyperlipidemia, hypertension, metabolic syndrome, and weight management. This professional will collaborate closely with the interdisciplinary team within a patient-centered medical home (PCMH) framework. The role involves working alongside Health Educator Nurses and Medical Assistants to enhance UDS quality improvement initiatives focusing on Body Mass Index (BMI). This position is crucial to the team-based primary care model that promotes coordinated, patient-centered, and comprehensive health services.
Key Responsibilities
- Conduct comprehensive nutrition assessments using the nutrition care process, recommend suitable nutrition therapy interventions, and document care plans in the electronic health record.
- Deliver nutrition education and health promotion initiatives to clients and support the Health Education team with preventative strategies.
- Effectively communicate and coordinate relevant patient information among the care team to ensure optimal patient care.
- Assess and educate patients regarding nutrition-related medications, dietary supplements, and potential drug-nutrient interactions.
- Develop, write, and review educational materials related to nutrition and preventive health services in accordance with local standards.
- Provide training to staff on nutrition education guidelines as necessary.
- Refer patients to appropriate interprofessional team members and resources as needed.
- Assist in the preparation of required reports and conduct quality assurance in line with federal guidelines.
- Collaborate with clinical teams for data collection, health outcomes reporting, and program evaluation.
- Engage in continuous professional development through training and conferences.
Essential Qualifications
- A four-year college degree in Dietetics or a related field, with a preference for two years of experience in community or clinical nutrition education or completion of a dietetic internship with RD/RDN credentials.
- Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentials from the Commission on Dietetic Registration.
- Certification in CPR and AED for Health Care Professionals through recognized organizations.
- Documented excellence in customer service with the ability to instruct individuals at various skill levels.
- Demonstrated competency in nutrition education methods and techniques.
- Proficiency in Microsoft Office applications and relevant program-related software.
- Experience working with diverse populations and cultures.
- Familiarity with motivational interviewing techniques.
- Valid Arizona driver's license and current auto insurance.
- Bilingual (English/Spanish) is strongly preferred.
