Job Opportunity

We are seeking an experienced and compassionate Medical Social Worker to join our Hospice team at Medical Services of America. As a key member of our interdisciplinary team, you will provide essential social work services to patients and families in need.

Responsibilities
  • Collaborate with healthcare professionals to assess and address the social and emotional needs of patients and families.
  • Develop and implement individualized care plans to support patients and families in coping with illness and loss.
  • Provide bereavement counseling and support to patients and families as needed.
  • Serve as a liaison between patients, families, and community resources to ensure seamless care coordination.
Requirements
  • Master's or Bachelor's degree in Social Work from a Council on Social Work Education (CSWE) accredited program.
  • Current licensure to practice as a social worker in the state of Georgia.
  • Minimum one year of experience in hospice care or a related field.
  • Valid driver's license.
