About the Role

">

The Palliative Care RN Case Manager will be responsible for delivering high-quality care to patients utilizing the nursing process of assessment, interventions, implementation, and evaluation. You will contribute to an interdisciplinary group to enhance quality of hospice care and case manage all assigned patient clinical records.

Key Responsibilities

  • Assume primary responsibility for case management of all assigned patients by assessing and evaluating the patient's condition, controlling symptoms, and observing any issues regarding the patient and family. Reports any issues or changes to hospice staff and physician as needed.
  • Provide direct care to patients as prescribed in the interdisciplinary plan of care in order to maintain the highest level of comfort and quality of life.
  • Coordinate community resources and other agency disciplines participating in patient care.
  • Complete comprehensive patient assessments to identify needs and provides appropriate care as supported by documentation and patient care plan.
  • Teach primary caregiver(s) how to safely operate equipment, such as suction machines, Hoyer lifts, nebulizers, oxygen tanks/concentrators, bedside commodes, and other physical equipment as needed.
  • Responsible for implementing patient care to support terminally ill patients and families in personal homes or nursing facilities.
  • Knowledge of referral and admission processes, to include evaluation and educational visits that may require explaining the hospice/Medicare benefit.
  • Assess home care needs, being aware of physical, emotional, and spiritual aspects and gather data on social, economic, and cultural factors which may influence health, well-being, and quality of life.
  • Provide patient and family teaching as needed.
  • Provide support to families at time of death.
  • Assist patients, family members, or other clients with concern and empathy.
  • Respect confidentiality and privacy.
  • Communicate in a courteous and respectful manner.
  • Attend and participate in interdisciplinary group meetings.
  • Comply with all Federal, State, Local laws, and abide by Medicare Conditions of Participation guidelines as set forth by Center for Medicare & Medicaid Services (CMS).
  • Uphold the mission, vision, and values of SouthEast Hospice.
  • Develop & maintain community relationships and always present a positive image.
  • Perform as a member of the SouthEast Hospice team as a whole and participate in the total quality management philosophy of the agency.
  • Operate under the supervision of the Patient Care Manager and/or RN Administrator and under the Medical Director's orders.

Requirements

To be successful in this role, you must possess a valid license as a Registered Nurse in the state of Georgia (or reciprocal state), graduate from an accredited school of nursing, and have a minimum of one year of recent professional nursing experience. Additionally, you should have knowledge of physical assessment, completion of all pertinent documentation, and communication to the SouthEast team.
