Job SummaryWe are seeking a compassionate Hospice CNA to join our team. As a key member of our care team, you will provide personalized care and support to patients in their homes or facilities.
Responsibilities- Assist patients with daily living activities, such as bathing, dressing, and grooming
- Provide emotional support and companionship to patients and their families
- Document patient progress and report any changes or concerns to the interdisciplinary team
- Collaborate with healthcare professionals to develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Certified Nursing Assistant (CNA) license
- Minimum 6 months of patient care experience
- Current CPR certification
- Valid driver's license and automobile insurance
