Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify and assess patient and family needs, re-evaluating as necessary every 15 days.
  • Develop and implement individualized plans of care in collaboration with the interdisciplinary group.
  • Document patient progress, identifying goals, interventions, and responses to care.
  • Collaborate with patients, families, physicians, and other healthcare professionals to deliver comprehensive care.
Requirements
  • A minimum of one year of clinical experience as a Registered Nurse.
  • Current RN licensure in the state of practice.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and reliable transportation or access to public transportation.

Estimated salary: $62,000 - $80,000 per year, depending on location and experience. This estimate may vary based on factors such as cost of living, industry standards, and market conditions.
