Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at The Good Shepherd Community Clinic, Inc. As an LPN, you will play a vital role in providing high-quality patient care and promoting health outcomes in our community.

Key Responsibilities
  • Conduct comprehensive assessments of patients' health and psychosocial needs, developing treatment plans collaboratively with patients and healthcare teams.
  • Strive to empower patients, assisting and supporting them in adhering to treatment plans and self-management goals, while monitoring and documenting progress.
  • Collaborate with external service providers to plan and coordinate patient treatment, including managing referrals to outside agencies.
  • Record all interventions, phone, email, and in-person, in patient charts, compiling and producing reports on service delivery and outcomes.
  • Correspond with patients, families, and healthcare teams through personal contact, phone, email, and regular mail, returning messages and relaying information in a timely manner.
  • Work with senior leadership to obtain data for outcome measurement reporting.
  • Report patient progress in patient files and to medical staff.
  • Collect and prepare laboratory specimens, coordinating care with lab companies as needed.
  • Perform basic laboratory and point-of-care testing.
  • Schedule patients' appointments, lab, radiology, and outside agency services.
  • Perform and record vital signs on patients.
  • Prepare patients for medical procedures and assist with procedures.
  • Maintain patients' medical records.
  • Keep exam rooms sterilized and stocked with supplies.
  • Perform other cleaning duties as assigned.
  • Manage medication refills.
  • Work with patients of all ages, from newborn to geriatric, focusing on women's health, OB, immunizations, and more.
  • Communicate relevant information to medical, dental, wellness, pharmacy teams, and COO to maintain continuity of care.
  • Promote smooth interactions with other departments and foster good interdepartmental relations.
  • Work effectively with supervisors and all personnel in positions of authority.
  • Maintain cooperative working relationships with all personnel.
  • Consistently demonstrate ability to recognize and manage priorities.
  • Perform duties independently with minimal supervision and direction, showing initiative.
  • Maintain confidentiality and adhere to HIPAA guidelines.
  • Follow all safety rules.
  • Maintain a well-groomed appearance and a well-kept work area.
  • Manage multiple projects to conclusion.
  • Provide reports as requested by senior leadership.
  • Work collaboratively as a member of a team and promote teamwork with other staff members.
  • Provide compassionate, non-judgmental care to patients.
  • Adhere to and enforce GSCC Policies and Procedures.
  • Assess and report opportunities for cost savings and process improvements.
  • Exercise attention to detail and problem-solving skills.
  • Resolve issues in a timely manner.
  • Assume responsibility for timely reporting to senior management or regulatory agencies.
Requirements
  • Oklahoma Licensed Professional Nurse (LPN) license required.
  • Ability to communicate and work effectively with people of varying backgrounds, healthcare professionals, and support staff at all levels.
  • High level of problem-solving and critical thinking skills.
  • Respect and regard for the dignity of all patients and their families/caregivers.
  • Excellent computer skills required for internal communications.
  • Ability to work independently as well as part of a team.
