About the Role

We are seeking LPNs to coordinate care and provide direct nursing support to adults with intellectual and developmental disabilities in a safe, empowering, and satisfying work environment. The ideal candidate will possess excellent communication skills and be able to work collaboratively with our team.

Responsibilities

  • Perform bimonthly in-home audits and medication reviews.
  • Maintain accurate and up-to-date health records according to DDS standards.
  • Assess and coordinate healthcare services for individuals, initiating follow-up and communication of all medical concerns to the Director of Nursing and relevant parties.
  • Administer medications as needed, adhering to MAP regulations.
  • Provide G and J tube care when required.
  • Review ISPs for current knowledge of programming, self-medication, behavior plans, hygiene skills, and physical therapy plans.
  • Order medication from pharmacy as needed.
  • Meet with Director of Nursing for supervision and direction.
  • Attend staff meetings and present current medical/health issues to the team.


Requirements

  • Licenses & Certifications: Current MA license, Valid Driver's License.
  • Experience: New graduates welcome; at least one year of experience as an LPN is preferred.


Working Conditions

This full-time position offers a $10/hour differential pay rate and comes with a sign-on bonus. Benefits include health insurance, vision insurance, paid time off, and a retirement plan after 90 days of employment.

The estimated annual salary for this role is approximately $54,000 based on 32 hours per week.
