Job Summary

We are seeking a highly motivated and detail-oriented Community Outreach Coordinator to join our team at Health Plan San Joaquin. As a key member of our outreach team, you will play a critical role in educating the community about our program and services, establishing and maintaining relationships with community partners, and representing our organization at approved community meetings.

Key Responsibilities
  • Attend approved outreach events to educate attendees about our program and services
  • Develop leads by conducting pre-screening of prospective members for enrollment into our program or Medi-Cal application assistance
  • Conduct presentations to promote our services and engage with the community
  • Represent our organization at approved community meetings to network, identify potential partnerships, and visibility opportunities
  • Assist in event preparation, including setup and takedown
  • Receive and initiate calls to potential members to provide information, conduct initial eligibility, follow-up on leads, and for other reasons as required
  • Create required documentation, including call logs and other tracking systems
Requirements
  • High school diploma or general education degree
  • One year of experience in a similar position or equivalent combination of education and experience
  • Basic knowledge of the eligibility requirements and benefits of Medi-Cal, ACA, and other state-sponsored programs
  • Basic knowledge of the healthcare issues facing the underserved
  • Basic knowledge of community resources that assist the underserved
  • Strong interpersonal skills, including the ability to interact effectively with individuals both inside and outside of our organization
  • Strong oral and written communication skills, including the ability to communicate professionally to diverse individuals and groups
  • Ability to work independently and as part of a team
  • Ability to understand and apply straightforward documents, and oral and written instructions
  • Ability to compare and discern the difference between multiple sets of data or information
  • Time management and organizational skills, including the ability to use time effectively and efficiently
