POSITION OVERVIEW:
The individual in this role will be tasked with delivering comprehensive case management services, which encompass evaluating service requirements, planning care, and advocating for clients involved in transitional living programs.
KEY RESPONSIBILITIES:
Case Management Duties:
- Facilitate psychoeducational group sessions, case management initiatives, educational outreach, and referrals to relevant services and community resources as needed.
- Conduct initial and ongoing assessments of individual needs for case management services.
- Support clients during transitions between different levels of care.
- Develop and periodically update a case management plan that outlines service activities.
- Engage in communication, coordination, referral, linkage, and related activities.
- Oversee service delivery to guarantee client access to essential services.
- Advocate for patients, connecting them to physical and mental health care, transportation, and ensuring retention in primary care.
- Maintain client records in accordance with organizational policies and contractual obligations.
- Acquire and sustain a thorough understanding of available community resources and act as a liaison.
- Foster relationships with referring and supporting agencies and represent the organization at various community gatherings as required.
- Participate in facilitating ancillary services, such as continuing care, networking with community organizations, engaging in community events, and developing and updating referral and resource information.
ESSENTIAL QUALIFICATIONS:
To excel in this position, an individual must be capable of fulfilling the duties and responsibilities effectively. The qualifications listed below represent the necessary knowledge, skills, and abilities:
Experience and Skills:
- Basic proficiency in computer usage.
- Strong documentation capabilities.
- Able to manage multiple tasks simultaneously.
- Exceptional communication skills, both written and verbal.
- Culturally and linguistically sensitive to the populations served.
- Knowledge of substance abuse and treatment methodologies is preferred.
- Ability to exercise sound judgment and discretion.
- Ability to collaborate effectively within a team environment.
- Commitment to the highest ethical standards, particularly regarding confidentiality.
- Professional appearance and demeanor.
- Possession of a valid driver's license and ability to secure and maintain vehicle insurance as required.
- Successful completion of a criminal background check is mandatory.
- Compliance with any new requirements, including vaccinations mandated by local, state, federal, or funding agencies.
- CPR Certification, First Aid Certification, and an annual Tuberculosis test are required.
EDUCATION AND EXPERIENCE:
- Preferred two (2) years of experience in case management.
- Basic computer skills.
- Strong documentation skills.
- Able to manage multiple tasks effectively.
- Excellent written and verbal communication skills.
- Graduation from an accredited four-year institution with a degree in Psychology, Social Work, Rehabilitation Counseling, or a related field is preferred.
- A High School Diploma is required.
WORKING ENVIRONMENT:
- Work primarily occurs in an office or residential/outpatient treatment setting.
- Some outdoor activities may be necessary.
- Client UA testing is required.
- Ability to de-escalate situations within the program is essential.
