Kaiser Permanente is seeking a highly skilled Licensed Clinical Social Worker to join our team in Honolulu, Hawaii.
Job SummaryThis is a full-time position that requires a Master's degree in social work from an accredited college or university, a passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.
Key Responsibilities- Provides comprehensive case management and ambulatory behavioral health services, including assessment, evaluation, diagnosis, treatment planning, case management, and psychotherapeutic intervention for members in the outpatient setting.
- Conducts social work consultation, performs clinical diagnosis or psychotherapy, and provides psychotherapy and counseling for individuals utilizing a wide range of evidence-based interventions and modalities.
- Interviews members and evaluates their psychological and mental status, utilizing the latest Diagnostic and Statistical Manual of Mental Disorders (DSM) to diagnose patients.
- Assesses urgent or high-risk clinical situations and makes appropriate interventions, coordinates referral of patients to psychiatrists or other clinicians, and assists with admission to hospitals or other specialized facilities.
- Plans and provides services in the appropriate level of care, assesses needs for mental health treatment, and makes appropriate referrals as needed.
- Performs case management, coordination, and monitoring of social work service plans in areas of personal, social, or economic resources, conditions, or problems.
- Applies knowledge base, theoretical concepts, specific functional skills, and essential social values used to effect change in human behavior, emotional responses, and social conditions to help enhance or restore personal and social functioning.
- Participates in departmental functions, such as staff meetings, staff in-services, committees, clinical team meetings, departmental planning, and quality management activities as assigned.
- Maintains database and case management tools to monitor status and care gaps of members, participates in orientation of new personnel, and training of staff.
- Assesses patients in Emergency Departments on an as-needed basis in service areas where Kaiser Permanente has privileges to provide mental health assessments and crisis intervention.
- Participates in performance improvement, implements Quality Improvement processes, assists with development and implementation of departments' annual work-plan, performs clinical record audits, and maintains educational record and provides evidence of competency.
- Masters Degree in social work from an accredited college or university, passing score on the clinical level national board examination, and supervised clinical social work training, as required by Hawaii Revised Statutes (HRS) 467E-7.
